Forms

Required – Medical consent & prescription form Accordion Closed

ALL campers under 18 years old MUST submit this form in order to check-in on the first day of camp. All signatures must be present on this form for it to be considered valid. Please pay special attention to directions regarding sending medications with your camper.

Required – Permission to travel form Accordion Closed

This form is required for all campers.

Camper Showcase Application 2020 (previously “Honor recital”) Accordion Closed

Campers who wish to audition to perform in the camper showcase during camp must submit this completed form on the first day of camp. Please read the requirements carefully before submitting.

Technology release form Accordion Closed

This form is only for campers taking online course required by their school district.
