Required – Medical consent & prescription form Accordion Closed
ALL campers under 18 years old MUST submit this form in order to check-in on the first day of camp. All signatures must be present on this form for it to be considered valid. Please pay special attention to directions regarding sending medications with your camper.

This form is required for all campers.
Grand Canyon trip form (Senior camp only) Accordion Closed
Senior campers who have registered and paid the additional fee ($80) during the online registration process for the Grand Canyon trip on the middle Sunday of camp (Sunday, June 23, 2019) must also submit this form. Campers will leave early Sunday morning, hike/shop/tour the Grand Canyon, and return in time for dinner and evening activities that day. A bag lunch will be provided.
*Because we rely on numbers for the bus rental, this trip is non-refundable*
Camper Showcase Application 2019 (previously “Honor recital”) Accordion Closed
Campers who wish to audition to perform in the camper showcase during camp must submit this completed form on the first day of camp. Please read the requirements carefully before submitting.
