Applications may be mailed or dropped off. Applications that are mailed must be postmarked within this time frame. If you choose to drop off your application, Mrs. Smolinski is located in Health Sciences Building 66, Second Floor, Office 205. Please only submit the last three pages of the packet.
The application packet you turn in should only contain the following:
- Your completed application. Typed or written. Please only submit the last three pages of the packet.
- Official transcripts from every university and/or college you have attended. (NAU transcripts may be unofficial)
Although portfolios, pictures, resumes, colored paper, and reference letters are often included in other applications, these items are not considered as part of our process. Please do not include them.
Please DO NOT staple your application pages together.
Please DO NOT change the format of the application.
You may submit your application without your transcripts, but please make sure they are turned in no later than 01/31.
We do not accept emailed or electronic transcripts.

The application is accepted January 1st through January 31st.
Completion of the NAU Dental Hygiene Program application process by the deadline is the sole responsibility
of the applicant. Meeting deadlines and completing application information forms properly demonstrates the
ability, maturity, and self-motivation desired by NAU Dental Hygiene Department.
Transcripts Accordion Closed
NAU transcripts can be unofficial, you may print them off our your LOUIE Account.
You may submit your application separate from your transcripts. (if your transcripts are being mailed directly or they will not arrive in time)
We do not accept emailed or electronic transcripts.
You must submit one official transcript of your academic record from each college or university you have attended. The transcript can be considered official only if it is unopened in the institution’s envelope and stamped with the seal of the college or university.
Required Science prerequisites must not be older than 5 years, all other required prerequisites must not be older than 10 years.
The NAU Dental Hygiene program only accepts letter grades A, B & C for required prerequisites. Letter grades D & F for prerequisites are not accepted.
Applying to Northern Arizona University Accordion Closed
If you are accepted into the Dental Hygiene program and are not currently enrolled at the university you must also be admitted to Northern Arizona University before starting classes in the fall. This is a separate application procedure; contact Undergraduate Admissions for information and an undergraduate application.
Admission to the university is required in order to begin the Dental Hygiene program. You will need to send official transcripts to the university’s registrar. The registrar will not accept transcripts from the Dental Hygiene Department.
