Important Information Regarding COVID-19 & Our Clinic Accordion Closed
In alignment with Governor Doug Ducey’s declaration of a public health emergency for our state, Northern Arizona University continues to closely monitor the spread of coronavirus (COVID-19) and is addressing the outbreak in a proactive and comprehensive manner campus-wide, in coordination with federal, state, and local health and emergency preparedness organizations. The outbreak of COVID-19 is fluid and continues to warrant the attention and flexibility of each of us as we work together to keep our students, faculty, staff, and communities safe and healthy.
Considering this declaration, the NAU Dental Hygiene Clinic will be closed Monday March 23 until Friday April 3, 2020. However, the NAU Dental Hygiene Department will remain staffed during this time, Monday- Friday 8:00 am to 5:00 pm. The Clinic will resume operations on Tuesday, April 7 2020, unless otherwise notified. Notices about changes will be communicated on a continual basis.
Kiddie Clinic scheduled on Saturday March 28, 2020 has been rescheduled for Saturday April, 25, 2020. At this time and we would love to reschedule you for this date.

We are following protocols established by the Centers for Disease Control and Prevention (CDC), and we are actively collaborating across the NAU community to inform and protect our students, faculty, patients and staff. NAU will continue to follow recommendations from our public health and government partners. As we closely monitor the situation with COVID-19, please pay attention to your own health needs and take precautions to prevent the spread illnesses. 1. Stay home if you’re sick. 2. Avoid close contact with people who are sick. 3. Clean and disinfect frequently touched objects and surfaces. 4. Wash your hands. 5. If you haven’t already, get a flu shot. Many of the symptoms and risk factors in respiratory diseases are similar, and it is still flu season. 6. Cover your mouth when you cough or sneeze with your inner elbow.

Appointment Information & Times Accordion Closed
- Fall Semester: Wednesdays and Thursdays 1:00 pm and 5:00 pm
- Spring Semester: Tuesdays 8:00 am and 1:00 pm, Wednesdays and Thursdays 1:00 pm and 5:00 pm
- Appointments with Senior students are roughly 2.5 hours long.
- Appointments with Junior students are roughly 3 hours long and you will need several appointment to complete treatment.
- Please plan on attending more than one appointment to complete your treatment.
- Many patients will require multiple appointments and may see more than one practitioner depending on the treatment needed.
- 24 hour cancellation notice, anything after 24 hours will be considered a last minute cancellation
- After 3 last minute cancellations or no shows a patient will be dismissed from our clinic.

We now offer a flat fee that includes your cleaning, exam, x-rays, and fluoride.
- $65 for Adults
- $60 for NAU students, alumni, faculty, staff and senior citizens over the age of 65.
- $45 for children 13 and under
- Sealants are $5 per tooth
We accept Visa, Mastercard, cash and checks. Payment is due at time of check in.

Coconino County Preventative Dental Care Voucher Program Accordion Closed
Did you know, Coconino County has a Dental Hygiene Voucher program? If you qualify, you can receive FREE preventative dental hygiene services at our clinic! See if you qualify for FREE preventative dental care today!
Preventive Care Voucher: The Preventive Care Voucher is designed to assist adults in obtaining preventive dental care. The program is open to income qualified adults who either have no insurance, or who are on AHCCCS. How to Obtain a Preventative Care Voucher:
Present in person in Flagstaff to complete an application. If approved, vouchers will be issued at the time of application. You can also fill out the application and bring it with you to your appointment!
Proof of current Coconino County residency, which may include one of the following:
Welfare/child care records
Federal, State, or Local (municipal) records
Support payment records
Homeowner or tenant records
Utility bills (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
Financial (loan, credit, investments, etc.) records
Affidavit from homeless shelter
To verify income eligibility, one of the following items must be provided:
Letter from employer stating income
Ninety (90) days worth of check stubs from each job
Verification of Certification from WIC
Letter of eligibility from SNAP, TANF, or AHCCCS
Three months (90 days) documentation of unemployment, housing assistance, Social Security, disability, pension or veteran’s benefits/military income
Letters of financial support

- dental hygiene therapy (prophylaxis)
- conservative periodontal therapy (scaling and root planning and periodontal maintenance)
- fluoride application
- sealants (coating to prevent decay on the chewing surfaces of teeth)
- teeth whitening (Please see requirements below)
- Must be a patient with the NAU Dental Hygiene Clinic
- Must have had a teeth cleaning here within the last 6 months
- Whitening only available during second half of NAU spring semester each year
- We do not provide restorative dental services such as extractions, fillings, crowns, root canals, etc.

Please read our new patient information pamphlet. Please be aware, your treatment us most likely will take more than one appointment to complete. Some patients will require multiple appointments and may see more than one practitioner depending on the treatment needed.
Please complete your new patient forms prior to your appointment. After you have completed each form, please press the Submit button at the bottom of the page to automatically send us your information. On your first visit to the clinic, we will have your completed forms available for review and signatures. The security of your personal data is one of our primary concerns and we have taken precautions to protect it.

Coming from the South (Phoenix/Sedona): I-17 becomes Milton Rd when you enter town. From I-17 / Milton Rd., take the McConnell Dr. exit 341 on right. This exit appears as you first approach town. At the end of the McConnell Dr. exit, turn right at the stop sign on to McConnell Dr. Turn right again at the next, immediately appearing, three way stop sign, onto Pine Knoll Dr. (There is a large Northern Arizona University sign at this corner.) Go straight through the first 4 way stop, and at the second 4 way stop sign on Pine Knoll Dr. turn left into the Health Professions parking lot. You may also go past this stop sign and use the second entrance to our parking lot; you will now be headed directly toward our loading dock.
Coming from the North along Milton Rd. in Flagstaff: Turn east onto University Drive (there is a stop light at this intersection with a Target store on the NE corner and a shopping center with a KFC and an Arby’s on the SE corner). Turn right at the first traffic light, this will put you onto Knoles Dr. Proceed over the hill on Knoles Dr. until the stop sign at the bottom of the hill (McConnell Circle). Turn right on McConnell Dr. and go to the next stop sign (Pine Knoll Drive); turn left onto Pine Knoll Dr. Go straight through the first 4 way stop, and at the second 4 way stop sign on Pine Knoll Dr. turn left into the Health Professions parking lot. You may also go past this stop sign and use the second entrance to our parking lot; you will now be headed directly toward our loading dock.
Coming from I-40: take exit #341 – McConnell Dr. At the stop sign at the bottom of the off-ramp, turn right onto McConnell Circle. Persons with Disabilities Access
Please enter the front entrance at the west end of the Health Professions building. Turn right into the first hallway. At the end of the hall (east end), turn right; the elevator is now on your right (southeast end of building).
Parking Permit Information Accordion Closed
The university requires patients to have parking permits displayed in all vehicles. Please see our Parking Guidelines.
When you call to schedule your appointment, you will be given a parking pass kiosk code. The parking pass kiosk is located in front of the handicapped parking next to the Health Professions building. The code is only good for the duration of 1 appointment in the clinic. The parking pass must be placed on the passenger side of your dashboard.
Parking Service’s rules and regulations state that parking passes may not be given to NAU students, staff, faculty, or employees. We are sorry for the inconvenience.
