Animal use/IACUC forms
NAU faculty and any associated researchers must submit an animal use protocol and receive Institutional Animal Care and Use Committee (IACUC) approval before animals can be used in teaching or research. The IACUC ensures that the university complies with all state and federal laws and regulations.
Failure to comply could result in loss of federal funding for all animal-related research (not just in projects, but also for colleagues) and personal liability.
Forms to ensure compliance with regulatory requirements
- NAU IACUC Protocol form – for animal use protocol
- NAU IACUC annual review form – a principal investigator is required to submit a brief protocol summary annually
- Modification request form
- Occupational health form
- NAU IACUC final review form – after a three-year period, protocols expire and a protocol is required
- Adverse event reporting form
- Tissue transfer form – most vertebrate animal tissue coming to the university must be reviewed and approved by the IACUC prior to arrival
- Inter-institutional agreement – collaborating work done at sites other than NAU
