Tuition Reduction Partnership form

Process for standard partners

    1. Complete the Tuition Reduction Partnership form after you have been admitted to Northern Arizona University. This form must be submitted each semester in which you intend to enroll at NAU.
    2. Upload a copy of the following documents to verify your employment. (Acceptable File Formats: .pdf, .doc, .docx, .xls, xlsx, .rtf, .txt, .jpg, .jpeg, .png, .tiff, .tif.)
      • your driver’s license (first time only)
      • a recent employee pay stub (We only need to see your name and the current date on your paystub. Feel free to block all other information).
    3. The Tuition Reduction Partnership form must be completed and verified prior to the tuition and fees payment deadline for the intended semester of enrollment (https://in.nau.edu/sdas/important-payment-dates/).
    4. Submit this document to NAU no earlier than the dates listed below:
      • Fall Semester -July 1
      • Spring Semester -November 1
      • Summer Semester -April 1

Here are the Initial Submission Instructions for tuition reimbursement request:

  1. Fill out this Employer Verification Form and have your employer verify your employment.
  2. Complete the Tuition Reduction Partnership form after you have been admitted to Northern Arizona University (NAU). This form must be submitted each semester in which you intend to enroll at NAU.
  3. Upload a copy of the following documents to verify your employment. (Acceptable File Formats: .pdf, .doc, .docx, .xls, xlsx, .rtf, .txt, .jpg, .jpeg, .png, .tiff, .tif.)
    •  your completed Employer Verification Form (first term only)
    • your driver’s license (first term only)
    • current paystub (We only need to see your name and the current date on your paystub. Feel free to block all other information).
  4. The Tuition Reduction Partnership form must be completed and verified prior to the tuition and fees payment deadline for the intended semester of enrollment (https://in.nau.edu/sdas/important-payment-dates/).
  5. Submit this document to NAU no earlier than the dates listed below:
    • Fall Semester -July 1
    • Spring Semester -November 1
    • Summer Semester -April 1

HonorHealth

Follow these steps to process your tuition reimbursement request:

  1. Fill out the HonorHealth Employer Verification Form and have HonorHealth verify your employment (this is done for your first semester only).
  2. Upload a copy of the following documents to verify your employment. (Acceptable File Formats: .pdf, .doc, .docx, .xls, xlsx, .rtf, .txt, .jpg, .jpeg, .png, .tiff, .tif.)
    • your completed Employer Verification Form (first term only)
    • your driver’s license (first term only)
    • current paystub (We only need to see your name and the current date on your paystub. Feel free to block all other information).
    • Upload at (link to Onbase form)
  3. The tuition reduction form must be completed, verified and submitted to NAU prior to the payment deadline to reduce your tuition fees by 10%. However, we will accept your documentation up until the last day of the applicable term for the requested semester. In this circumstance, a refund will be issued to you for the 10% reduction in tuition. Payment deadlines can be found at  https://nau.edu/sdas/important_payment_dates/
  4. Submit this document to NAU no earlier than the dates listed below:
    • Fall Semester -July 1
    • Spring Semester -November 1
    • Summer Semester -April 1
