Anyone 12 years of age and older can register for a vaccination appointment. To make an appointment for a relative, use the patient portal. If you are registering a minor (12-17 years old) for the vaccine, please note that an adult must register on the AzDHS registration site before a minor account can be added and an appointment scheduled. All minors must be accompanied by an adult when receiving the vaccine, and a consent form must be signed.
You will need to bring an ID to the vaccine site for your appointment. The vaccine is free. Insurance is not required, but individuals with insurance should bring their card.
New appointment slots will be made available for the following week on Fridays at 11 a.m.
Information about all vaccination sites across Arizona can be found at the AzDHS find vaccine webpage. Anyone who needs help registering can call 844-542-8201 or 602-542-1000 to be connected with someone who can assist in English or Spanish.
The State of Arizona site at NAU is distributing the Pfizer vaccine under the guidance of Arizona Department of Health Services and in partnership with the Arizona Department of Emergency and Military Affairs, Coconino County and Northern Arizona Healthcare.
