Minors in laboratories at Northern Arizona University  

Northern Arizona University holds exciting learning opportunities for visiting minors. We welcome these visitors and make their safety our top priority.

Before a minor is eligible to participate in projects in Northern Arizona University laboratories or animal facilities, the Principal Investigator (PI) or her/his designee must make sure the following forms are reviewed and submitted to the Office of Environmental Health and Safety.

The Principal Investigator (PI) or her/his designee must review and submit:

• Northern Arizona University minors policy
• minors research proposal

The minor's parent/legal guardian must review the following form and submit to the Insurance Officer at NAU Property and Liability Insurance Services, P.O. Box 4067.

• parental/legal guardian consent form

Pre-planning is key to this process as an EHS site visit will be required prior to project initiation. The PI or designee must also conduct and document site/project-specific training of the minor(s) prior to the commencement of the respective project.
