Animal use/IACUC forms  

Northern Arizona University faculty and any associated researchers must submit an Animal-Use Protocol and receive Institutional Animal Care and Use Committee (IACUC) approval before animals can be used in teaching or research. The IACUC ensures that the university complies with all state and federal laws and regulations.

Failure to comply could result in loss of federal funding for all animal-related research (not just in projects, but also for colleagues) and is personal liability.

The following forms will help ensure compliance with regulatory requirements:

• Submit an animal use protocol using the Animal Use Protocol Form.
• A principal investigator is required to submit a brief protocol summary annually using the Annual Review form.
• Modify a protocol that was previously approved by submitting a Protocol Modification Request.
• After a three-year period, protocols expire and a protocol Final Review is required.
• Most vertebrate animal tissue coming to the university must be reviewed and approved by the IACUC prior to its arrival using the tissue transfer form.
• Collaborating work done at sites other than NAU may require an Inter-Institutional Agreement.
