Appointments

Call 928-523-8995 or sign-in to our online service to schedule an appointment. Once you are logged in, you can also:

  • complete your health history
  • view lab  results
  • send and receive secure messages
  • complete visit questionnaires
  • request prescription renewals
*Please Note: If you make an appointment and are unable to make it you, you will need to cancel the appointment. If you do not cancel the appointment in an appropriate amount of time you will be charged a "no show" fee that varies depending on the type of appointment that is made. Also, if you are running late please call (928) 523-8995 to inform us that you will be late. If you are more than 10 minutes late to  your appointment there is a change you will not be able to see the provider and will be charged the "no show" fee.

Worker's compensation

We also offer complete health care to Northern Arizona University employees and student employees injured in the University workplace. We will help you complete all required paperwork.

Same-day and walk-in appointments are available for worker's compensation cases.
