Get your birth control from Medical Services
Complete these steps to get your first
prescription for hormonal birth control at Medical Services.
Step one: decide which birth control is best for
you
If you don’t know what kind of birth
control you want to use, visit the Association of Reproductive Health
Professionals site to choose a birth control method.
Step two: attend the online birth control class
Begin the online birth control education class. Pay close
attention to the general information and sections that apply to
you. This class will take up to an hour.
Step three: read and sign the informed consent
form
After you’ve read about your type of
birth control, read the informed consent form thoroughly and:
print, sign, and date two copies
keep one copy for your records
bring the other copy to Medical Services on the day of your appointment
Step four: complete your online health history
Fill out your online
health history if you haven’t already.
Step five: make your appointment
Call 928-523-8995 to make an
appointment with Medical Services to get your birth
control. If you also need an annual well woman exam, let the staff know when you make your
appointment.
Step six: go to Medical Services
On the day of your appointment,
bring your signed informed consent form. Arrive at least 15 minutes early to sign in at the front desk
and complete a health history questionnaire if you haven’t done
it online. Bring any questions you want answered about your
appointment, exam, or contraceptive.
