Adviser forms

Each academic year a student’s adviser evaluates the student’s academic progress using the continuing student evaluation form.

A student who exhibits weakness in their academic progress is required to complete a Professional growth plan.  

Program forms

All graduate students start out in the comprehensive examination plan. Students in the MS in Mathematics program may apply to change to the Thesis plan. Students completing their degree in the comprehensive examination plan need to schedule their oral examination. The Graduate College has checklists of requirements for both the thesis and comprehensive exam options.
