Grand Canyon Trip Form (Senior Camp Only)

Senior campers who have registered and paid the additional fee ($80) during the online registration process for the Grand Canyon trip on the middle Sunday of camp (Sunday, June 29) must also submit this form as well as the Permission to Travel Form [below]. Campers will leave early Sunday morning, hike/shop/tour the Grand Canyon, and return in time for dinner and evening activities that day. A bag lunch will be provided. *Because we rely on numbers for the bus rental, this trip is non-refundable*

Honor Recital Application

Campers who wish to audition to perform in the student honor recital during camp must submit this completed form no later than June 20, 2014. Please read requirements carefully before submitting.

ALL campers under 18 years old MUST submit this form in order to check-in on the first day of camp. All signatures must be present on this form for it to be considered valid.

Payment Voucher

A payment voucher may be included with any payments not made electronically. This is an optional form.

Permission to Travel Form

This form is required for all campers who are signed up to go to the Grand Canyon. Additionally, any camper who intends to leave campus at any time during camp must have this completed form on file with the Camp Office.
